Whether they are embedded within your practice or working from our call center, our Care Team Coordinators will act as an extension of your team. Embedded CTC’s are hired from the communities served, and all telephonic communications into an area occur from a local telephone number. Our CTC’s are trained in comprehensive patient engagement strategies and excel at getting patients to your office for needed care.


We recognize that your practice has multiple payer types. Unlike programs that focus strictly upon Medicare patients, we offer truly payer agnostic programs so that you no longer have to stratify patient care based upon the insurance they carry.


There are NO up front fees or fixed costs associated with our services. Stated simply, if we do not create NEW VALUE for your practice, we are paid nothing.


Our processes have been designed to minimize disruption to your normal daily/weekly routines. Most clients report that our processes are easily integrated into existing clinic work flows, and that minimal or no changes are required from providers.

Our process is NON-DISRUPTIVE to the daily workflow in an office, and results in improved healthcare for the Patient, lower overall costs to payers, and improved QUALITY METRICS and additional revenue to Providers. We do this in a 100% “pay for performance” business model (we are only paid if we create new income for the clinic), and we target 2X PHYSICIAN NET INCOME in 12 to 24 months!!
